Trouble with Auto responder code

I use Traffic Wave and no matter what I do the opt in box does not show up on the website. Can you help with this please. I have sent a help request for this before but did not get an answer.

Thank you for the help that has worked to a point. But I still get "webformcode" showing on the site but it is not in the script or in the traffic wave data.

you have the word webformcode. do you also see your optin boxes now?
